Customer verification
A clear record of your application, both required signatures, and Advanced Virology’s review. Verification is required before shipment; the new online process brings these steps together.

Prepare your application
Sign in and provide your institution, authorized end user, responsible official, and delivery details. The account holder and authorized end user may be different people.

Review the complete form
Review every page of the personalized Customer Verification form and delivery details before submitting. If anything changes, create a corrected revision and review it again.

Both required people sign
The authorized end user and the institution’s responsible official each review and sign the same submitted revision through their own secure access. The responsible official may be the designated biosafety officer or another authorized institutional official.

Advanced Virology reviews
An authorized AV reviewer checks the completed application and signature evidence, then records approval, requests corrections, or declines the application. Your record shows the decision and any next steps.
Need verification before online activation?
Download the existing Customer Verification form, obtain both required signatures, and email the completed form to our team. We will review your submission and help with account setup.
